Pay: $23 - $30 / hour

Job Description
The Community Health Worker will serve as a key member of the clinic care team, supporting a dedicated patient panel in a fast-paced primary care environment. This role is responsible for building meaningful relationships with patients and families, helping them navigate the healthcare system, and supporting preventative, chronic, and acute care initiatives.
Key Responsibilities
- Manage and support an assigned panel of patients to ensure care plan adherence and follow-up
- Provide education and resources to help patients make informed health and lifestyle decisions
- Identify and help address social determinants of health impacting patient outcomes
- Collaborate with providers, nurses, medical assistants, and care coordinators to support patient care efforts
- Build rapport with patients during clinic visits and community interactions
- Document patient interactions accurately within electronic systems
- Support preventative, chronic, and acute care initiatives for senior patient populations
- Assist patients with navigating healthcare services and community resources as needed
Qualifications
- A minimum of 1 year of Community Health Worker experience within a clinical healthcare setting is required
- A Community Health Worker (CHW) certification is required
- Bilingual in Spanish and English is required
- Experience working with adult and geriatric patient populations is required
- Bachelor’s degree preferred
- Strong technical skills and comfort navigating multiple systems and technology platforms
